Please complete the follows forms to report a student's absence, tardy or request for an early dismissal.
Note: Please have your Photo ID ready at pickup
** Medical excuses are required for any absences after the 10th parental excuse as per Pennsylvania Compulsory Attendance Law and must be sent to the office.
** Forms received after a students 10th parental excuse will be accepted, however, it will be marked as illegal absence.
** Excuses will only be accepted up to 3 days after the student's absence. Please send them to the office within the designated timeframe.

Vacation Request Forms:
This form should be used when requesting time off from school for any reason other than regular excused absences (illness, death in family, impassable roads, appointments, emergencies personally affecting the child).
Please observe the following instructions in the order listed below:
The Parent / Guardian MUST complete this request form.
The student must submit the request to the Attendance Specialist for approval PRIOR to leaving for the vacation. (drivardo@mpasd.net)
Vacations are approved/denied based on the student's current attendance record. Vacation days count and are included in the student's 10 allotted school year absences.
You will only be notified by a school official if your request is denied.
The Mt. Pleasant Area School District does not encourage the taking of vacation during the school year. However, in the event that no other time for vacation is possible, the following policy will be in effect. It will be the responsibility of the student to make up all missed work due to an approved vacation. The vacation period shall not extend beyond FIVE school days. Vacations WILL NOT be approved during PSSA/Keystone Testing, or the last two weeks of school.
