Immunizations
Children IN ALL GRADES need the following:

4 doses of tetanus (1 dose on or after the 4th birthday)
Usually given as DTP or DTaP or DT or Td

4 doses of diphtheria (1 dose on or after the 4th birthday)
Usually given as DTP or DTaP or DT or Td

4 doses of polio (1 dose on or after the 4th birthday and at least six months after previous dose given)

2 doses of measles
Usually given as MMR.

2 doses of mumps
Usually given as MMR.

1 dose of rubella (German measles)
Usually given as MMR.

3 doses of hepatitis B
-
2 doses of varicella (chickenpox) vaccine or history of disease
Children ENTERING 7th grade need the following (Tdap is recommended to be given between the ages of 11 and 12):

1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
- 1 dose of meningococcal conjugate vaccine (MCV)
Children Entering 12th grade need the following:
- 1 dose of meningococcal conjugate vaccine (MCV)
- If one dose is given at 16 years of age or older, that shall count as the 12th grade dose.
